Filler: the most widely used products for wrinkle correction and are available as injectable preparations based on hyaluronic acid.

During aging, our body undergoes numerous changes, the most visible of which occur on the skin.
Over the years, a smaller amount of collagen and hyaluronic acid is produced, causing the loss of elasticity of the skin.
Thus, furrows and folds begin to develop visible on the skin surface: wrinkles.
Chronic exposure to sunlight and smoke are the main causes of skin aging.

Wrinkles can be classified into dynamic wrinkles and static wrinkles: dynamic wrinkles become more evident when the muscles are active;
static wrinkles are noticeable when the face is at rest or not in motion.
There are several areas of the face involved, such as the frontal region, the lips and the perioral area, the cheekbones, the chin.

How are wrinkles treated?

There are several treatments currently available, depending on the type, severity and characteristics of the skin to be treated.
Before resorting to surgery, aesthetic medicine treatments can be carried out in our clinic by plastic surgeons Dr Cappellina Diego and Dr Cappellina Cesare.

Hyaluronic acid is a disaccharide normally present in our skin, responsible for its elasticity and hydration.
When this element decreases with age, wrinkles and trophism of the skin occur.
The hyaluronic acid filler is injected subcutaneously through special syringes equipped with fine needles: the injections are normally performed on an outpatient basis.

 

Is it a painful treatment?

The treatment with filler can be performed on an outpatient basis. The discomfort perceived during the treatment is quite bearable.
To overcome this drawback, we inject hyaluronic acid after having anesthetized the part to be treated with a very effective anesthetic ointment, which makes the treatment much less annoying.

Which areas can be treated?

Filler is indicated in the following circumstances:
-Shape expression lines, crow’s feet, glabellar wrinkles (which originate in the forehead, precisely in the portion immediately above the nose) in these cases it can be associated with botulinum toxin
-increase thin lips without volume
– Volumize cheekbones

How long does the filler treatment last?

The regions to be treated must be carefully planned together with the patient. The duration of the treatment varies from about 10 to 20 minutes, depending on the areas to be treated.
Before the treatment, an anesthetic ointment is placed, which, left to absorb for about 15/20 minutes, allows the injections to be carried out in the anesthetized regions so as not to be painful. Following the treatment, it is useful to administer ice for the first few hours, to decrease the risk of hematomas.

 


How long does the effect last?

The volumizing effect of the filler is immediate (compared to the botulinum whose effect starts after a few days).
In the first 48 hours, small hematomas may be visible, especially in areas of the face rich in vessels, such as the lips and nose. Small irregularities may also be visible, due to the swelling present in the treated areas.
If irregularities persist, corrections can be made to restore the symmetry of the result
The duration of the correction varies from 6 to 10 months. Following the injections of hyaluronic acid, our body is stimulated to produce endogenous hyaluronic acid, consequently with the repetition of the injections the quantity of drug to be injected can be less.

 


What are the alternatives to filler?

Since their introduction, hyaluronic acid injections have proved to be the first choice for outpatient facial rejuvenation and volume increase and correction treatments.
A recent alternative to their use is lipofilling, that is, the adipose tissue graft taken from your body, especially from the abdominal region, hips, inner knees.

Fat is collected as in a normal liposuction, but unlike this, the fat is treated and purified and is ready to be injected into the areas to be treated. Lipofilling is effective around the lips, the nasogenic sulcus, the cheekbones and can be performed simultaneously with other surgical procedures, such as lifting.
Such treatment can be permanent. However, the response to lipofilling is individual; in some patients fat can take root 100%, in others vice versa it can be reabsorbed.
It is a safe surgical procedure if performed by qualified personnel and in a suitable environment.
It is slightly more invasive than treatment with hyaluronic acid.

 

PRP

An alternative available in our ambulatory is PRP: plasmine rich plasma: a blood sample is taken from the patient; the sample is then treated by a centrifuge to obtain a platelet rich plasma concentrate. It is performed for the purpose of biorevitalizing the patient’s skin, multiple sessions may be necessary. It is also indicated in the preventive treatment of baldness.

BOTULINUM TOXIN


Botulinum toxin is a drug that allows you to temporarily inhibit the action of the facial mimic muscles by reducing the formation of expression lines.

In Italy the use of botulinum toxin was authorized by the Ministry of Health in 2004 only for one type of drug called Vistabex (in Italy), Botox (in the United States) or Vistabel (in France), and only for the forehead wrinkle treatment.

Vistabex by Allergan is the most widely used botulinum toxin drug in Italy

It is essential that the preparation is injected by a specialist in plastic surgery, dermatology or maxillofacial surgery.
Botulinum toxin is indicated for the treatment of expression lines (dynamics) of the upper part of the face (forehead and periocular region, the so-called “crow’s feet”
The treatment is performed on an outpatient basis. Local anesthesia is not necessary, as the needle with which the injections are performed is very thin and does not cause pain to the patient.

Injections are performed at the mimic muscles of the forehead, eyebrow and periocular areas.
After the treatment, it is advisable to rest from physical activities for 24 hours, to avoid that the increase in pressure does not favor the spread of the drug.

Unlike filler, whose filling effect is immediate, the botulinum takes a few days to act.
The maximum effect is reached after about 15 days.
The duration of the effect is approximately six months, after which the result is completely reversible

 

VITAMINS


Revitalizing vitamins are liquid products that are injected immediately under the dermis to promote the stimulation of collagen synthesis and improve the turgor and elasticity of the skin. There are various types of revitalizers ranging from liquid hyaluronic acid to those with vitamins and trace elements of protein precursors.

The procedure, on an outpatient basis, consists in releasing the gel immediately below the wrinkle epidermis (surface layer of the dermis) with a very thin disposable needle (diameter of a hair).
The total duration does not exceed approximately 30 minutes.
Normal professional and family life can be resumed immediately after treatment.
At the end of the treatment, there may be a slight redness and small wheals in the injection sites for a few hours, which can spontaneously resolve within a few hours.

It is possible to treat all wrinkles on the face, neck and décolleté, inner thighs and inner arms.
Usually a cycle of 3/4 injections is proposed to the patient
